This includes the following classes:
- ActiveModel::Serializers::Xml
- ActiveRecord::Serialization::XmlSerializer

Remaining are `limit`, `precision`, `scale`, and `type` (the symbol
version). These will remain on the column, since they mirror the options
to the `column` method in the schema definition DSL

The previous tests were passing, because nothing ever looked at the
generated XML. What was previously being generated was
`<firstname type="NilClass">...`, which is not consistent with all other
cases where there is not a known type.

When generating xml with a custom implementation of serializable_hash,
if using the :except option, it was being overriden by the default AR
implementation that attempts to ignore the inheritance column from STI
automatically. So, if you have an implementation like this:
def serializable_hash(options={})
super({ except: %w(some_attr) }.merge!(options))
end
The :except option was correctly being used for :json generation, but
not for :xml, because the options hash already contained the :except
key with the inheritance column, thus overriding the customization.
This commit fixes this problem by removing the :except logic from the
xml serializer, that happened before calling serializable_hash. Since
serializable_hash also does the same check for inheritance column, this
logic was duplicated in both places, thus it's safe to remove it from
xml serializer (see ActiveRecord::Serialization#serializable_hash).
This is an attempt to solve issue #2498, that claims for a
"Single transformation API for both xml and json representations".

XmlMini define the xml 'datatime', but according to
http://www.w3.org/TR/xmlschema-2/#dateTime could be better
change this to 'dateTime' with upper case letter 'T.
So 'DateTime' and 'Time' are redefined from 'datetime' to 'dateTime'

The following constants were renamed:
ActiveModel::Serialization => ActiveModel::Serializable
ActiveModel::Serializers::JSON => ActiveModel::Serializable::JSON
ActiveModel::Serializers::Xml => ActiveModel::Serializable::XML
The main motivation for such a change is that `ActiveModel::Serializers::JSON`
was not actually a serializer, but a module that when included allows the target to be serializable to JSON.
With such changes, we were able to clean up the namespace to add true serializers as the ArraySerializer.

This commit moves support for the :include serialization option for
serializing associated objects out of ActiveRecord in into ActiveModel.
The following methods support the :include option:
* serializable_hash
* to_json
* to_xml
Instances must respond to methods named by the values of the :includes
array (or keys of the :includes hash). If an association method returns
an object that is_a?(Enumerable) (which AR has_many associations do), it
is assumed to be a collection association, and its elements must respond
to :serializable_hash. Otherwise it must respond to :serializable_hash
itself.
While here, fix #858, XmlSerializer should not singularize already
singular association names.

Also, refactored ActiveModel serializers to just use ActiveSupport::XmlMini.to_tag. As consequence, if a serialized attribute is an array or a hash, it's not encoded as yaml, but as a hash or array.

All internal Rails JSON encoding is now handled by ActiveSupport::JSON.encode(). Use of #to_json is not recommended, as it may clash with other libraries that overwrite it. However, you can recover Rails specific functionality
if you really want to use #to_json.
