- #30980 introcuded a change to not use `Arel.star` when model have ignored columns, a query used to look like `SELECT *. FROM developers` whereas now it would like `SELECT column1, column2 FROM developers`
- If a column has the same name has a reserved database specific keyword (such as key, where ...) then the query would fail because the names aren't quoted

This regression was caused at 213796fb due to polymorphic predicates are
combined by `Arel::Nodes::And`. But I'd like to keep that combined
because it would help inverting polymorphic predicates correctly
(e9ba12f7), and we can collect equality nodes regardless of combined by
`Arel::Nodes::And` (`a AND (b AND c) AND d` == `a AND b AND c AND d`).
This change fixes the regression to collect equality nodes in
`Arel::Nodes::And` as well.
Fixes #31338.

Fix sqlite migrations with custom primary keys

Previously, if a record was created with a custom primary key, that
table could not be migrated using sqlite. While attempting to copy the
table, the type of the primary key was ignored.
Once that was corrected, copying the indexes would fail because custom
primary keys are autoindexed by sqlite by default.
To correct that, this skips copying the index if the index name begins
with "sqlite_". This is a reserved word that indicates that the
index is an internal schema object. SQLite prohibits applications from
creating objects whose names begin with "sqlite_", so this string should
be safe to use as a check.

This is a regression since Rails 4.2.
SQLite3 integer is stored in 1, 2, 3, 4, 6, or 8 bytes depending on the
magnitude of the value. Assuming default valid value as 4 bytes caused
that actual valid value in INTEGER storage class cannot be stored and
existing value cannot be found.

Actually SQLite3 doesn't have JSON storage class (so it is stored as a

In 35836019, class level `update` and `destroy` suppressed

has a compatibility problem. The previous behavior should not be
changed.

There are two concerns which are both being combined into one here, but
both have the same goal. There are certain attributes which we want to
always consider initialized. Previously, they were handled separately.
The primary key (which is assumed to be backed by a database column)
needs to be initialized, because there is a ton of code in Active Record
that assumes `foo.id` will never raise. Additionally, we want attributes
which aren't backed by a database column to always be initialized, since
we would never receive a database value for them.
Ultimately these two concerns can be combined into one. The old
implementation hid a lot of inherent complexity, and is hard to optimize
from the outside. We can simplify things significantly by just passing
in a hash.
This has slightly different semantics from the old behavior, in that
`Foo.select(:bar).first.id` will return the default value for the
primary key, rather than `nil` unconditionally -- however, the default
value is always `nil` in practice.

Since #31173, mysql2 adapter depends on `automatic_close` which is
introduced since mysql2 0.4.3. So the adapter with the mysql2 version
before doesn't work with fork now.

This changes `StatementTimeout` to `QueryCanceled` for PostgreSQL.
In MySQL, errno 1317 (`ER_QUERY_INTERRUPTED`) is only used when the
query is manually cancelled.
But in PostgreSQL, `QUERY_CANCELED` error code (57014) which is used
`StatementTimeout` is also used when the both case. And, we can not tell
which reason happened.
So I decided to introduce new error class `QueryCanceled` closer to the
error code name.

Since #31129, new error class `StatementTimeout` has been added.
`TransactionTimeout` is caused by the timeout shorter than
`StatementTimeout`, but its name is too generic. I think that it should
be a name that understands the difference with `StatementTimeout`.
